  • Original Birth Certificate
  • Immunization Record
  • 2 Proofs of Residence**

Two of the following listed documents must be provided as Proof of Residence:

  • Consumers Bill or Water Bill
  • Drivers License
  • Voter Registration with address matching address on Drivers License
  • Current Lease Contract/Formal Rental Agreement
  • Most Recent Property Tax Statement
  • Property Purchase Agreement

*Kindergarten children must turn 5 by September 1, 2015.
